Appropriate attire must be worn to all dance classes; please see Lisa about the uniform. Danceworks will make decisions as to the appropriate class level for each student and consult students or parents/caregivers if a change is appropriate.

I release Danceworks Ltd of any liability for accident or injury that may occur while attending dance classes. I am responsible for the drop off and pick up of my child at the appropriate times.

You will be asked to sign this in person once classes begin.
